Social Services: Improved Responsiveness

  1. Social Services has additional Social Workers who are assisting with coverage of the acute care floors. There will now be two Social Workers assigned to each floor to allow them to be familiar with patients and accessible to staff and providers. The Health Unit Coordinator can assist providers in locating the assigned Social Worker at any time. New Social Worker contact numbers are now posted at all nursing stations.
  2. Social Services is able to issue prescription vouchers to indigent patients to assist with timely discharge. A referral can be made to Social Services, which will coordinate with UMC Pharmacy. The voucher will allow the patient to be provided with seven days of a prescription medication; unless it is an antibiotic. Then, the entire prescription would be filled. Social Services has three Social Workers available during the weekend from 8AM - 5PM. They are able to help with medication assistance. There is also a Social Worker on-call after hours, 5PM - 8AM, who can be contacted by calling the UMC operator at 806.775.8200. The Health Unit Coordinator can assist in contacting both the weekend Social Worker and on-call Social Worker via the on-call schedule found on the UMC intranet. 
  3. Effective July 1, 2015, Social Workers will be on their nursing unit from 8AM – 11AM, Monday – Friday to round with provider teams, attend huddle, and be available in CPOE rooms or in nurse stations, so they can join patient discussion and discharge processing as needed. Social Workers will be calling providers with questions related to discharge planning needs in order to plan ahead so patient’s discharges are not delayed, and patient flow is not impeded.
